Nguyen is the Sino-Vietnamese reading of the Chinese surname Ruan (阮), named for the ancient State of Ruan during the Shang Dynasty and an ancient four-stringed plucked lute. During the first millennium AD, when northern Vietnam was governed as a province of imperial China, administrative officials systematically assigned Chinese-derived hereditary surnames to Vietnamese clans, with Nguyen becoming one of the most prominent aristocratic lineages.

Over subsequent centuries, Nguyen expanded through historical political realignments. Following the collapse of the Ly Dynasty in 1225 and the Mac Dynasty in 1592, defeated royalty adopted the Nguyen surname to escape retribution. Its preeminence was sealed with the establishment of the Nguyen Dynasty (1802–1945) by Emperor Gia Long, during which many imperial subjects were rewarded with or voluntarily assumed the royal family name.
